    New Position Request Form

  • FLSA Non-Exempt - Employee must be paid hourly and hours tracked, must be paid for all hours worked.

    FLSA Exempt - Employee is exempt from overtime only if they meet qualifications for meeting specific exemptions as outlined by federal or state FLSA, they are paid a weekly amount, should be paid in whole day increments, not necessary to track hours.

  • If pay type is Hourly, please provide an hourly rate. 

    If pay type is Salary, please provide an annual salary.

    If pay type is Commission, please provide an estimated annual salary.

  • For purposes of federal immigration law, employees are required to provide documentary evidence of their identity and eligibility for employment in the United States. You may upload either:

    • One document from List A; or
    • One document from List B in combination with one document from List C

    In certain cases, they can present an acceptable receipt for List A, B, or C documents. The validity of receipts varies by document so the employee must keep their employer updated on the status of the replacement document. An employer representative will need to review the original document upon receipt to update section 2 of the Form I-9.
